Accelerate Health Equity is proud to share our latest impact report. Since the launch of our initiative, we’ve made great strides in expanding our partnerships and taking stock of the health equity landscape in our city. Through these efforts, we have gained a keen understanding of the issues we’re best equipped to tackle alongside our partners. Over the last year, AHE has identified four key areas where our work can make the most impact in dismantling health inequities: Access to Care, Social Determinants of Health, Chronic Disease and Injury Prevention, and Maternal, Infant, and Child Health. By honing our focus, we have built a more concrete roadmap and made progress in our goal to achieve a healthier Philadelphia. Most notably, we have begun to see the impact of our efforts on the lives of Philadelphians.
AHE has used the last year to build coalitions, as well as support work in schools, hospitals, and within communities to address key health inequities in our city, including those related to gun violence, heart disease, behavioral health, and colorectal cancer. You’ll see highlights of our work in these areas throughout the report, in addition to the official launch of our Social Determinants of Health Subcommittee and the creation of a new health access coalition that joins West Philadelphia community organizations and health systems together to bolster access to care. As we look ahead, we will continue to create spaces for information sharing, communication, and collaboration between AHE institutions, including within AHE’s own subcommittees and Steering Committee.
There’s always more to be done, but we are committed to building more opportunities to collaborate and inviting new players to ensure our work is inclusive, scalable, and sound. Through our work, we want to show how health systems, communities, and citizens can work together for the overall health of a city. In our Spring 2025 Impact Report, you will find brand new projects that promote access to care, key research findings that can inform interventions to dismantle inequities in patients’ healthcare journeys, and updates on our grantees who set out to curb gun violence in our city. We hope this report provides a small insight into the innovation and change that is possible, and highlights what is on the horizon for our initiative and the impact that we can achieve together.
